ATHLETIC PHYSICALS/WEIGHT ROOM PERMISSION FORMS

In order to use the Weight Room, the student must provide a completed History , Physical Examination and a Medical Eligibility Form. The link to the forms are below.

All REGISTRATIONS for the 2024-2025 SPRING SESSION should be completed online at FamilyID. The FamilyID Spring Session Registration will be open:

FEBRUARY 5, 2025 - MARCH 3, 2025. Medical Eligibility forms must be submitted to the school's office before the registration deadline : MONDAY, MARCH 3, 2025.

The NJSIAA start dates for all SPRING SESSION IS : MARCH 10, 2025

If you are having difficulties with scheduling an appointment with your own primary care physician, you can make an appointment to go to the facility of our school physician, American Family Care - Urgent Care.  Appointments must be scheduled between February 17, 2025- February 28,2025 in order for the cost of the physical to be covered by the district.

Requirements for Participation in Hanover Park Regional High School District Athletics:

All students who participate in the athletic program are required to obtain a physical examination as per Hanover Park Regional District Policy. 

A new physical is required every 365 days. -

Step 1: Parent/Guardian and student-athlete must complete the FamilyID registration process.

(IF PHYSICAL IS NOT CURRENT/UP TO DATE /VALID)

A CURRENT/UP TO DATE/VALID PHYSICAL IS ONE THAT HAS BEEN PERFORMED BY A MEDICAL PROFESSIONAL WITHIN 365 DAYS AND HAS BEEN CLEARED BY THE SCHOOL PHYSICIAN. 

Step 2: The History Form and Physical Examination Form must be completed by a health care provider and DO NOT NEED TO BE SUBMITTED TO MAIN OFFICE. Both forms should be kept by the healthcare provider who completes them. ONLY THE COMPLETED MEDICAL ELIGIBILITY FORM SHOULD BE SUBMITTED TO THE SCHOOL.The school physician will review the Medical Eligibility Form upon receipt. The school district will then provide the parent with written notification signed by the school physician stating approval and not approval in the student's participation in school athletics. If you have been diagnosed with asthma or carry an epipen, additional forms must be signed by your physician and returned with your completed Medical Eligibility Form.

--> If you have been diagnosed with asthma, print a copy of the Asthma Action Plan and return to the Health Office.

 --> If you are required to carry an Epi Pen, print a copy of the Epi Pen Form and return to the Health Office.

If this is your first time using FamilyID/Arbiter Sports, you will need to create your family  account. You will use this account for all student athletes' athletic experiences.

 FamilyID/Arbiter Sports Registration Steps:

  1. Click on the blue "CREATE ACCOUNT" link in the top right hand corner of FamilyID.com.

  2. Create your secure FamilyID/Arbiter Sports account by entering your name, email address twice, a password twice, and checking the "I agree to the FamilyID/Arbiter Sports terms of Service" box. Then select the green 'Create Account' button.

  3. You will receive an email with a link to activate your new account. (If you do not see the email, check your spam or junk folders.)

  4. Click the link to activate your account.

  5. This will bring you to the FamilyID/Arbiter Sports Dashboard.

PLEASE NOTE THAT YOU MUST USE THE LINK THAT SAYS WHIPPANY PARK HIGH SCHOOL ATHLETICS TO REGISTER FOR A SPORT.

  1. After creating a FamilyID/Arbiter Sports account and following the email verification link or logging on as an existing user, you will be brought to your dashboard. Please type WHIPPANY PARK HIGH SCHOOL in the search box that reads “Type organization name or keyword to find a program”.

  2. Find the Whippany Park Athletic Program and select the blue link with the program’s title to head to the athletic registration page.

  3. The program's landing page will show a description of the program with a green ‘REGISTER NOW’ button located below. You can either select the green ‘REGISTER NOW’ button or just simply scroll down the page until you see the blue header that says 'SELECTIONS' to begin your registration process.

  4. At any time, you may login to www.familyid.com or https://arbiter.io/ to update your information and to check your registration.
