All REGISTRATIONS for the 2024-2025 WINTER SEASON should be completed online at FamilyID. The FamilyID Winter Session Registration will be open:

October 16, 2024 - November 4, 2024. Medical Eligibility Form must be submitted to the school's office before the registration deadline : November 4, 2024.

The NJSIAA start dates for all WINTER SPORTS are as followed:

November 25, 2024 (First official practice date) for Ice Hockey and Swimming.

November 25, 2024 (NJSIAA Tryout dates) for Basketball and Wrestling.

December 2, 2024  (First Official Practice) for Winter Track and Cheerleading.

Step 1:The History Form and Physical Examination Form must be completed by a health care provider and DO NOT NEED TO BE SUBMITTED TO MAIN OFFICE. Both forms should be kept by the healthcare provider who completes them. ONLY THE COMPLETED MEDICAL ELIGIBILITY FORM SHOULD BE SUBMITTED TO THE SCHOOL.The school physician will review the Medical Eligibility Form upon receipt. The school district will then provide the parent with written notification signed by the school physician stating approval and not approval in the student's participation in school athletics. If you have been diagnosed with asthma, diabetes, or carry an EpiPen, additional forms must be signed by your physician and returned with your completed Medical Eligibility Form. These forms can be found and printed off the FAMILYID website.

Step 2: Register on Family ID ( Must be completed for each season you play a sport or weight room participation)

FamilyID REGISTRATION STEPS:

  1. Click on the blue "CREATE ACCOUNT" link in the top right hand corner of FamilyID.com.

  2. Create your secure FamilyID account by entering your name, email address twice, a password twice, and checking the "I agree to the FamilyID terms of Service" box. Then select the green 'Create Account' button.

  3. You will receive an email with a link to activate your new account. (If you do not see the email, check your spam or junk folders.)

  4. Click the link to activate your account.

  5. This will bring you to the FamilyID Dashboard.
